Before your first consultation, we will ask you to complete a New Patient Form, which you can do at your convenience. If you have a male pelvic problem, the staff at Adelaide Urology Care may ask you to complete some questionnaires, which you can complete online here.

We kindly request that you please bring the following things with you:

• GP referral letter

• Medicare card

• Pension, DVA and Health Fund information

• Blood and urine test results

• Reports of any medical imaging studies (X–rays, Ultrasound scans, CT scans, MRI scans, bone scans, PET scans)

• List of medications

• Correspondence from previous specialists including letters and operation reports
